Utah Advance Directives - Living Will & Power of Attorney

There may come a time when you or your loved one is no longer able to communicate your preferred treatment.   For instance, would you want to be resuscitated after a massive heart attack or kept alive on a breathing machine no matter what?  Advance Directives, including a Living Will and a Health Care Power of Attorney, are the legal documents designed to clearly communicate the type of treatments desired and who has the right to make your healthcare decisions if you are incapable of doing so.  See and print Advance Directives.

Print Living Will and Power of Attorney 

A Living Will clearly defines your wishes concerning treatment and care intended to sustain life when you are no longer able to make or communicate your own decisions.  If you wish, this document may include a directive to physicians to withhold or withdraw life-sustaining procedures under certain circumstances called a DNR or Do Not Resuscitate order (Learn More).

The Power of Attorney document allows you to appoint someone called a "health care representative or agent" to make health care decisions for you when you are unable to communicate your wishes.  This representative cannot make decisions for you when you are capable of making them yourself.

Physician Order for Life Sustaining Treatment (POLST, or Life with Dignity Order):  The POLST form directs the medical team regarding the care and treatment you want provided or withheld.  It is almost always completed in response to a specific health threat.  Because it is filled out with the medical team during a immediate health threat, this form is not available for print.
