Why use Telemnoitoring?           

Our telemonitoring system allows First Choice Home Health & Hospice to evaluate a patient's medical condition on a daily basis. This enables our nurses to effectively and properly monitor patients so we can spot small problems and treat them appropriately before they become big problems. Telehealth Monitoring Systems provide a way to monitor patients and their needs within the comfort of their own homes.

As needed the telemonitor will:

• Take your vital signs
• Check your weight
• Assess your oxygen saturations
• Monitor your blood pressure
